Physician Consultants Business Solu seeks a Registered Nurse to lead the day-to-day operation of a home dialysis training facility and supervise LPNs and Patient Care Technicians delivering care to adult/geriatric ESRD patients.
The RN will perform home hemodialysis and peritoneal dialysis procedures, develop and manage individualized care plans, educate patients and families, and coordinate with the interdisciplinary team to ensure continuity of care.
Home Dialysis Training Nurse
Responsible for the day-to-day operation of the dialysis home training facility and for the oversight and function ofLicensed Practical Nurses and Patient Care Technicians providing the direct and indirect nursing care needs of anassigned of home patients primarily of an adult/geriatric ESRD patient population. Serves as leader, role modeland mentor to direct care personnel providing support, instruction, and direction in the delivery of dialysis therapy.This RN is ESRD Dialysis experienced in and able to perform home hemodialysis and peritoneal dialysis treatmentprocedures in accordance with facility policy. Utilizes nursing process to assess individual patient needs,determine, direct, and implement a plan of care, evaluate and revise plan according to changing patient careneeds. Initiates and provides on-going patient and family education and includes patient/family in care planning,using appropriate forms to document care provided. Coordinates the efforts of members of the interdisciplinaryteam to ensure continuity of care for each patient.
