Physician Consultants Business Solu is seeking a Registered Nurse to join our team in Macon, GA. The RN will manage and create care plans for home dialysis patients, including overseeing LPNs and patient care technicians, and ensuring adherence to facility policies.
The role emphasizes patient-family education, coordination of care, and ongoing clinical assessment to optimize outcomes for adult/geriatric ESRD populations in the home setting.
Home Dialysis Training Nurse Responsible for the day-to-day operation of the dialysis home training facility and for the oversight and function of Licensed Practical Nurses and Patient Care Technicians providing the direct and indirect nursing care needs of an assigned of home patients primarily of an adult/geriatric ESRD patient population. Serves as leader, role model and mentor to direct care personnel providing support, instruction, and direction in the delivery of dialysis therapy. This RN is ESRD Dialysis experienced in and able to perform home hemodialysis and peritoneal dialysis treatment procedures in accordance with facility policy. Utilizes nursing process to assess individual patient needs, determine, direct, and implement a plan of care, evaluate and revise plan according to changing patient care needs. Initiates and provides on-going patient and family education and includes patient/family in care planning, using appropriate forms to document care provided. Coordinates the efforts of members of the interdisciplinary team to ensure continuity of care for each patient.
