Job summary

Responsibilities

• Assess patients’ physical, social, and psychological status and collaborate with the interdisciplinary team to develop an individualized plan of care.

• Implement and coordinate care by directing the interdisciplinary team and ensuring compliance with guidelines and licensure provisions.
• Evaluate patient outcomes and revise the care plan as needed while documenting care and maintaining professional standards.
