Job summary

An established healthcare provider in Bronx, NY is seeking a Per Visit Physical Therapist HHA to conduct comprehensive assessments for clients and create care plans. Applicants must hold a valid New York State Licensure in Physical Therapy and have eight years of experience in a medical setting. A driver's license is required to facilitate home visits. This role offers a competitive hourly rate of $43.85.

Qualifications

• Graduate of a program in Physical Therapy approved by the American Physical Therapy Association.
• Current New York State Licensure.
• Two (2) years at senior level in field or acute medical hospital.
• Two (2) years supervised clinical experience in a medical setting preferred.

Responsibilities

• Conducts a comprehensive assessment, including physical, emotional, and mental status for clients of all ages.
• Develops plans of care with realistic outcomes, based on interpretation of assessment of data and consideration of client's preferences.
• Performs designated discipline interventions according to agency policies.
• Provides client/SO education on prevention or management of illness, functional disability.
• Evaluates the results of clinical interventions to measure progress toward goals.
• Plans sequence of home visits taking into account acuity and geographical location.
