Per Diem Speech Therapist - Sebastian & Barefoot Bay

Responsible to the Director of Professional Services and Clinical Manager to provide medically prescribed, goal-oriented therapy in accordance with the Physician Plan of Care, Company Policies, and professional Standards of Practice. Supervises personnel to deliver care and coordinates care delivery.

JOB RESPONSIBILITIES

1. Participates in the development of patient plans of care.
2. Makes visits to patients for assessments and evaluations and communicates with the physician as needed. Implements the Plan of Care.
3. Documents the care and submits electronic documentation in a timely manner.
