Job summary

BAYADA Home Health Care is seeking an experienced Speech Language Pathologist (SLP) for a per diem home health role serving adult and geriatric patients in Leesburg/Ashburn, VA. The SLP will join a multidisciplinary team to deliver rehabilitative care in senior living communities.

Responsibilities include home visits, comprehensive evaluations, treatment planning, ongoing care plan adjustments, and patient/caregiver education.

Qualifications

• Must hold VA licensure as an SLP or be eligible for licensure, with CCC-SLP preferred.
• Certificate of Clinical Competence in Speech-Language Pathology (ASHA CCC-SLP) or equivalent qualifications.
• Eligibility for ASHA examination or ongoing supervised experience toward certification.

Responsibilities

• Make home visits to clients in designated Leesburg/Ashburn, VA territory.
• Perform evaluations and provide therapeutic interventions for speech, language, swallowing disorders.
• Develop and adjust individualized treatment plans based on evaluations and physician orders.
• Educate patients and caregivers to promote health, safety and independent living.
• Document observations and interventions using an EMR system.
