Job summary

Apple Rehab Avon in Connecticut seeks a Speech Language Pathologist to join our team in a 60-bed skilled nursing and short-term rehab setting. We emphasize a family-like culture and professional growth for our staff.

The role reports to the Rehabilitation Director and focuses on quality resident care, evaluation, and treatment to maximize functional abilities while promoting positive interactions with residents and staff. LTC experience is preferred.

Qualifications

• Graduate of an accredited SLP program with CCC or CFY, and licensure eligibility.
• Prior long-term care experience preferred.

Responsibilities

• Works with Rehabilitation Director to evaluate and treat residents.
• Develops, implements, and documents resident care plans.
• Manages caseload in coordination with staff; maintains productivity.
• Instructs residents, families, and caregivers in speech therapy strategies for discharge.
• Ensures documentation/billing complies with policies and PPS guidelines.
• Provides supervision and education to affiliating students and rehab techs.
• Maintains a safe, clean, and compliant treatment environment.
