Responsibilities

• Evaluation and planning: perform initial evaluation, develop and implement treatment plans, involve patient and family, re-evaluate progress toward goals.
• Documentation: maintain complete documentation of evaluations, treatment, progress and discharge status.
• Collaboration: participate in rehab team, counsel family, coordinate care with MDs and assist in discharge planning.
