Collaborates with physicians, nurses, internal and external case managers, community agencies, and other health care professionals to implement safe, effective and timely patient care. Assignments vary based on department and service needs. Reports to the social work manager/manager of clinical service area.

Job Description

Essential Responsibilities: Provides clinical services to patients and families that address personal and environmental issues in order to maximize emotional, social, and physical well-being and the appropriate and efficient use of health care resources. Screens and triages patients who request service, are referred, or meet screening criteria for identifying patients' needs. Conducts psychosocial assessments and provides services as indicated. Partners with the multidisciplinary team to facilitate continuity of care planning. Meets productivity standards and expectations as set by the leadership in Social Work and the assigned clinical area. Provides counseling and psychotherapeutic services appropriate to setting. Provides timely and appropriate communication and documentation in the medical record. Provides consultation and education regarding psycho-social issues affecting patient care. Advises and addresses ethical and risk management issues. Presents and/or discusses clinical work in formal and informal settings, multi-disciplinary and clinical rounds and seminars. Participates in department or hospital process improvement and CQI efforts as needed. Partners with advanced staff on research. May presents clinical material and theory at teaching conferences and seminars Staff licensed as an LICSW may supervise staff or student interns and Serves as a content resource for multidisciplinary staff; and mentor/trainer to less experienced staff. Encourages an environment of idea sharing and feedback. Seeks opportunities and implements goals for own professional development.

Required Qualifications

Master's degree in Social Work Experience with computer systems required, including web-based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint, OneDrive and SharePoint.

Preferred Qualifications

Relevant health care and/or human service experience.

Age based Competencies

Employees in this job must be competent to provide patient care to the following age groups: Young adult: 16-30 years, Middle Age: 31 - 60 years, Elderly: 61+-.

Physical Nature of the Job

Light work: Exerting up to 20 pounds of force frequently to move objects. Some elements of the job are sedentary, but the employee will be required to stand for periods of time or move throughout the hospital campus.

Department Specific Job Responsibilities

Come and join BIDMC's overnight SW primary response Team! Currently we have a wonderful group of 4 social workers on this service, and we are looking to round it out to 5. This team covers the Emergency Department and the Center for Violence Prevention & Recovery pagers overnight from 11pm-7am on weeknights and 9pm-7am on weekends. It is a role in which these pagers are covered remotely (hopefully you are sleeping!) until there is a request for any urgent SW support, at which time there are some cases that can be responded to via remote work, and some cases that will require you to come in for an onsite response - there is a corresponding pay scale for different types of work. There is also a seasoned SW supervisor assigned every shift who can be reached for extra assistance and consultation. Requirements to serve in this capacity include: having an MSW degree and being licensed at an LCSW or higher the ability to arrive at the hospital within 45 minutes of being paged if clinically necessary a familiarity with the Emergency Department, or willingness to learn (paid training/shadowing provided) Being on-call,on average, one regularnight/week and rotating weekend nights/month. (The current openingisfor every Sunday + rotating with the group for Fri/Sat) Screens and triages patients who request service, are referred, or meet screening criteria for identifying patients' needs. Conducts psychosocial assessments and provides services as indicated. Partners with a nurse case manager and the multidisciplinary team, assists in discharge planning and/or outpatient continuity of care planning.

Pay Range

$33.64 - $54.33 The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
