Job summary

OUTREACH Rehabilitation in Manhattan, NY is seeking a Speech-Language Pathologist (Per Diem) to evaluate and treat adult and geriatric patients in home and community settings, focusing on communication, cognition, swallowing, and functional independence.

The role emphasizes collaboration with patients, caregivers, and physicians to develop individualized plans of care that support safe and meaningful outcomes. Prior home-based rehab experience is preferred and flexible scheduling is offered.

Qualifications

• Master’s Degree in Speech-Language Pathology from an accredited program.
• Minimum 1 year of experience working with older adults preferred.
• Dysphagia management and swallowing rehabilitation experience preferred.
• Cognitive-communication rehabilitation and neurogenic communication experience desired.
• Home-based rehabilitation experience preferred.

Responsibilities

• Perform comprehensive speech, language, cognitive, voice, and swallowing evaluations
• Develop and implement individualized treatment plans
• Provide speech, cognitive, communication, and dysphagia treatment
• Recommend compensatory strategies and therapeutic interventions
• Educate patients, caregivers, and family on home programs and carryover strategies
• Collaborate with physicians, caregivers, and interdisciplinary team members
• Complete timely and accurate electronic documentation in compliance with standards
