Job summary
Stand Out Home Care is seeking an RN to join our field team in New Jersey. This per diem role requires traveling to clients’ homes to deliver hands-on nursing care and to conduct initial assessments for tailored care plans.
Responsibilities include ongoing reassessments, leading caregiver orientations, and ensuring compliant documentation in collaboration with our administrative team. Applicants should hold an NJ RN license and CPR certification, with travel willingness a must.
Qualifications
- Active RN license in New Jersey.
- Active CPR certification.
- Experience in home care, hospice, or community health a plus.
- Willingness to travel to client locations.
- Strong clinical judgment and communication skills.
- Ability to work independently.
Responsibilities
- Conduct comprehensive initial assessments to establish tailored care plans.
- Perform regular reassessments to ensure quality and effectiveness of care.
- Lead field orientations for caregivers to ensure quality service.
- Collaborate with administrative team to maintain compliance and documentation.
- Travel to clients' homes across New Jersey to provide hands-on nursing care.
