Job summary

Sarah Care Adult Foster Care in Massachusetts is seeking a Licensed RN or LPN for per diem, part-time work. You will conduct health assessments, develop care plans with physicians and caregivers, and ensure regulatory compliance for AFC services.

Qualifications

• Current Massachusetts RN license in good standing.
• Minimum of 1 year of nursing experience, preferably in long-term care, adult foster care, or similar settings.
• Knowledge of Massachusetts Adult Foster Care regulations and healthcare laws.
• Ability to train and educate caregivers on proper care techniques and health monitoring.

Responsibilities

• Conduct initial and ongoing assessments of members’ health status and medical needs.
• Develop and implement care plans with physicians, caregivers, and other healthcare professionals.
• Monitor member well-being and respond promptly to changes in health.
• Provide education, training, and ongoing support to caregivers on proper healthcare practices.
• Ensure compliance with Massachusetts regulations for AFC services, including licensing and documentation.
• Maintain accurate medical records, including assessments, progress notes, and care plans.
• Collaborate with the clinical team to ensure high standards of care and service delivery.
• Perform other duties as assigned by the Director of Nursing or agency management.
