Job summary

Sarah-Care-Adult-Foster-Care in Massachusetts seeks a compassionate Registered Nurse to join our AFC team. You will provide clinical oversight, complete member assessments, support caregivers, and monitor health for adults living safely at home.

Qualifications

• Active MA RN license required.
• CPR certification required.
• Minimum 2 years’ experience with elders or adults with disabilities.

Responsibilities

• Complete annual nursing assessments and clinical evaluations.
• Develop and update AFC Plans of Care.
• Monitor member health status and report changes to PCPs.
• Conduct required home visits.
• Review caregiver documentation and care logs.
• Provide caregiver orientation and ongoing training.
• Complete nursing progress notes and required reports.
• Collaborate with Care Managers and interdisciplinary team.
