GREENFIELD Per Diem Registered Nurse or Licensed Practical Nurse PER DIEM NURSES! No nights, no weekends, and flexible scheduling! LifePath provides support services to older people, people with disabilities, and caregivers. We are currently seeking per diem RNs/LPNs for our Personal Care Attendant Program (PCA), supporting people of all ages in Franklin, Hampshire, Hampden, and the Montachusett Region of Worcester County.

The PCA Program Nurse is responsible for the performance and completion of clinical assessments of individuals who receive MassHealth PCA Services. Assessments occur in-person in the individuals' homes. When possible, nurses complete assessments in regions near their home-base. Per diem rates are $32/hr for LPNs and $34/hr for RNs. • Ability to work independently and as part of a team
• Knowledge of disabilities, assistive devices, and community resources
