Job summary

Affinity Hospice Care Inc. seeks an experienced Registered Nurse to support patients at home, reviewing care plans, coordinating with clinical staff, and ensuring high-quality hospice services in patients' homes.

This per diem role pays $140–$150 per visit and requires CA RN license, 1–2 years home health experience, OASIS coding knowledge, strong communication, and reliable transportation.

Qualifications

• Active Californian RN license in good standing.
• 1–2 years home health clinical experience.
• Working knowledge of OASIS and ICD-10 coding.
• Familiar with federal & state licensure requirements.
• Strong communication and keen attention to detail.
• Ability to work independently with minimal supervision.
• Proficient in electronic health record systems.
• Current CPR certification (AHA).
• Valid driver’s license and reliable insured vehicle.

Responsibilities

• Review OASIS assessments for completeness and compliance with regulations.
• Ensure accurate ICD-10 coding and sequencing based on patient conditions.
• Partner with clinical staff to resolve documentation and data issues.
• Monitor visit utilization and flag financial risks or underutilization.
• Identify and communicate trends affecting outcomes and process measures.
• Conduct OASIS inter-rater reliability visits with clinical team.
• Participate in Quality Improvement and Corporate Compliance activities.
• Stay current with best practices through ongoing education.
• Perform other duties as required for patient services.
