Job summary

Affinity Hospice Care Inc. is seeking a Registered Nurse to provide compassionate, high-quality home hospice care. You will help patients heal, stay safe, and thrive in their own environment, with teamwork and empathy at the core.

The role is per diem, offering $140–$150 per visit. Responsibilities include reviewing OASIS, ensuring accurate ICD-10 coding, collaborating with clinical staff, and staying current with best practices to support patient services.

Qualifications

• Active Californian RN license in good standing with CPR certification.
• 1–2 years of home health experience preferred.
• Knowledge of OASIS and ICD-10 coding required.
• Familiarity with federal/state regulations and licensure.

Responsibilities

• Review OASIS assessments for completeness and compliance.
• Ensure accurate ICD-10 coding and sequencing.
• Collaborate with clinical staff to resolve documentation issues.
• Monitor visit utilization and flag financial risks.
• Identify trends impacting outcomes and processes.
• Conduct inter-rater reliability visits with the team.
• Participate in Quality Improvement and Compliance activities.
• Maintain ongoing professional development.
• Perform other duties as required for patient services.
