Description:

Per diem Nurse to cover call outs:

Work at least 4 shifts per month if needed:

Starting compensation: $45.00 per hour.

If you prefer the flexibility to work as needed vs being scheduled and don't mind if there are not any shifts for you to pick up at the time, then this position is for you!

Responsibilities:

  1. Explains and coordinates patients' rights and advanced directives; explains Medicare Hospice Benefit and Level of Care Changes when indicated. Understands Local Coverage Determination Guidelines.
  2. Implements the complete nursing process for assigned patients and their families/representative including teaching appropriate care, treatment, information and health practices.
  3. Facilitates appropriate admissions and initiates plan of care for each patient which includes identification of nursing diagnosis, identification of goals with appropriate time frames, the design and implementation of appropriate interventions, and the outcome of care rendered.
  4. Provides hands-on patient care, education and teaching, medications, services, and prescribed treatments including those requiring professional nursing skills.
  5. Acts as charge nurse/administrator in absence of the administrator.
  6. Communicates with the physician, other health team personnel and significant others, and reports changes in the patient's physical and/or emotional condition and patient needs for continuity of care in an effective and professional manner.
  7. Ensures all chart audits are complete in a timely manner per regulations.
  8. Instructs and supervises the activities of hospice aides, Unit/Dietary clerk.
  9. Responsible for coordination of schedules; maximizes efficiency of patient care through monitoring of staff assignments based on acuity.
  10. Serves as a customer service advocate, facilitating rapid response to all referrals and patient needs.
  11. Communicates with Intake Coordinator to prioritize admissions on a daily basis for the Home for Hospice. Ensures all patients admitted to the Residence meet the appropriate criteria for Hospice services per Medicare guidelines.
  12. Demonstrates appropriate infection control measures when dealing with home and inpatient residents.
  13. Adheres to employment policies as described in the employee handbook; maintains and respects confidentiality; adheres to all HIPAA regulations.
  14. Completes all documentation, during shift, according to Medicare requirements.
  15. Participates in all required in-service education programs and meetings.
  16. Demonstrates compliance with the dress code policy by appearing well-groomed, wearing ID badge while on duty and maintaining a professional appearance.
  17. Performs other duties as assigned.

Position Requirements:

Required Skills/Abilities:

  • Excellent clinical skills to support inpatients and their families at the Home for Hospice. Possess the ability to provide basic and skilled nursing care, set priorities of care, and appropriately assess limits of interventions.
  • Understanding of the death and dying process.
  • Understands quality improvement programs to help identify opportunities for improvement, problem solving and implementing recommendations.
  • Possess strong verbal and written communication skills; demonstrates the ability to communicate effectively with patients, families, and team members regarding care issues.
  • Demonstrates strong organizational, time management, and prioritization skills.
  • Maintains the strictest confidence of all patient information and discloses information and data only to persons authorized by patient/family/representative or Hospice personnel.

Education and Experience:

Required Education: Diploma, Associate or Bachelor of Science in Nursing; BSN preferred. Licensed to practice as a Registered Nurse in the State of New Jersey. CPR/AED certification, and Serv-Safe if required.

Required Experience: Minimum one year of clinical experience in an acute setting.

Preferred Experience: Prior hospice experience preferred.
