Job summary

Affirmed Home Care is seeking qualified Registered Nurses to perform per diem assessments and on-call nursing visits in patients’ homes, serving all counties in South Jersey. Visits may be accepted based on the nurse’s availability and existing commitments.

This opportunity offers flexible scheduling and a chance to earn additional income while remaining in the home health field. Clinicians will be supported by a responsive clinical team and access to resources as needed.

Qualifications

• Active and unrestricted RN license in good standing.
• Previous nursing experience required.
• Home care or field nursing experience preferred.
• Strong assessment and documentation skills.
• Ability to work independently in patients' homes.
• Reliable transportation.
• Current CPR or BLS certification.

Responsibilities

• Complete start-of-care assessments.
• Conduct supervisory nursing visits.
• Perform patient reassessments.
• Complete post-hospitalization visits.
• Provide on-call nursing visits as assigned.
• Assess patients’ clinical conditions and care needs.
• Report changes in patients’ conditions to the clinical team.
• Complete all required visit documentation accurately and on time.
• Maintain patient confidentiality and follow HIPAA requirements.
• Comply with Affirmed Home Care policies and applicable nursing regulations.
