Job summary

Affirmed Home Care is seeking qualified Registered Nurses for per diem assessments and on-call visits in patients' homes across South Jersey. Enjoy flexible scheduling and the opportunity to earn additional income while maintaining your current role.

Visits are offered based on patient needs and your availability, with a supportive clinical team and the chance to work independently in a field-based setting. CPR/BLS certification and a current RN license are required.

Qualifications

• Active and unrestricted Registered Nurse license.
• Previous nursing experience required.
• Home care or field nursing experience preferred.
• Strong assessment and documentation skills.
• Ability to work independently in patients' homes.
• Excellent communication and time-management skills.
• Reliable transportation.
• Current CPR or BLS certification.

Responsibilities

• Complete start-of-care assessments.
• Conduct supervisory nursing visits.
• Perform patient reassessments.
• Complete post-hospitalization visits.
• Provide on-call nursing visits as assigned.
• Assess patients' clinical conditions and care needs.
• Report changes in patients' conditions to the clinical team.
• Complete all required visit documentation accurately and on time.
• Maintain patient confidentiality and follow HIPAA requirements.
• Comply with affied home care policies and applicable nursing regulations.
