All Metro Health Care, a Modivcare Personal Care Service, is seeking a Per Diem RN Clinical Supervisor to support patients and home health aides across Liverpool and nearby communities. The role is field-based with flexible, visit-based scheduling, and pay per visit with mileage reimbursement.

Qualifikationen

• Active New York State RN license in good standing.
• Minimum of 1 year RN experience in a skilled-care setting.
• Current CPR certification.
• Valid New York State driver's license and reliable transportation.
• Ability to travel throughout Liverpool and surrounding areas.

Aufgaben

• Conduct initial nursing assessments, reassessments, and supervisory visits in patients' homes.
• Supervise, evaluate, and provide clinical guidance to home health aides.
• Develop and update individualized care plans with physicians and the care team.
• Educate patients and caregivers on nursing care and needs.
• Monitor patient conditions and communicate changes to the care team.
• Maintain accurate clinical documentation.
