Part-Time RN Case Manager (Growth to Full-Time)
Schedule: Monday–Friday | 8:00 AM – 4:30 PM | No Nights or Weekends
Location: Wesley Chapel, FL & Surrounding Areas

All Homecaring Florida, a family-owned and rapidly growing home health agency, is expanding in Florida and looking for a compassionate and dedicated RN Case Manager to serve clients in Wesley Chapel and surrounding suburbs. This role begins as part-time (20–25 hours/week) with the potential to transition into a full-time position as your caseload grows.

What You'll Do:
  • Conduct comprehensive physical assessments and reassessments.

  • Develop and update personalized care plans.

  • Administer treatments and medications as prescribed.

  • Educate and support patients and their families.

  • Supervise and delegate tasks to HHAs/PCAs.

  • Mentor new nurses and participate in shared on-call rotation.

  • Communicate changes in patient condition and update documentation accordingly.

  • Collaborate with interdisciplinary teams and physicians.

  • Maintain compliance with Florida home health care regulations and documentation standards.

Qualifications:
  • Positive, caring attitude and commitment to ongoing learning.

  • Knowledge of OASIS documentation and CMS regulations.

  • Strong time management and case coordination skills.

  • Ability to perform comprehensive assessments, manage plans of care, and document timely.

  • Effective verbal and written communication.

  • Proficiency in using electronic health records, email, and basic technology.

  • Current FL RN license and AHA CPR certification.

  • Valid driver's license and auto insurance.

  • Recent negative TB test or chest X-ray.

  • Level 2 background screening (or ability to pass one).
