The Per Diem RN Case Manager (inpatient and/or ambulatory) is responsible for assessing, planning, organizing, and overseeing daily care coordination and transition activities to meet patients' complex health needs.

Requires knowledge of community health services, payer systems, and post-acute care for outpatient and inpatient populations.
Collaborates with clinic teams and inpatient case managers to ensure seamless care.
Integrates regulatory requirements, length of stay, resource allocation, funding, and capacity management to optimize service delivery.
Participates actively in departmental, multidisciplinary programs, committees, and special projects.
Works Monday through Friday days, with occasional weekend work up to three times per year.

To qualify, candidates need a BSN (Master's preferred), at least 5 years of nursing experience (pediatrics preferred), and a Certified Case Manager certification (preferred or to be obtained within two years).

Experience in discharge planning or utilization management is desirable.
