Qualifications

• RN License required
• Experience in case management preferred
• Current RN License in California

Responsabilités

• Develop individual care plans based on patient assessment and tests.
• Monitor and evaluate patient progress, updating treatment plans as needed.
• Recommend additional levels of care and therapy when medically indicated.
• Monitor appropriateness of therapeutic and rehabilitative care.
• Implement strategies to ensure patient and caregiver compliance with care plans.
• Provide education focusing on self-management for patients and families.
• Conduct conferences to identify patient risk factors.
• Facilitate patient return to daily activities with appropriate referrals.
• Develop treatment program procedures and clinical guidelines with the team.
• Educate inpatient case managers on transfer appropriateness to Skilled Nursing facilities.
