Job summary

California Rehabilitation Institute, a joint venture with Cedars, UCLA and Select Medical, is seeking a Case Manager - Per Diem in Century City/Los Angeles, CA. Schedule: 8am - 4:30pm. Rate: $66.00 per hour.

The Case Manager will coordinate health care decisions, discharge planning and care coordination across the continuum, with CCM certification preferred and licensure in state guidelines required.

Qualifications

• Current licensure per state guidelines in a clinical or related discipline.
• Bachelor's or Master's in health or human services.
• CCM certification preferred.
