Responsibilities

• Assess patients and develop a comprehensive case management plan.
• Coordinate plan of care with physicians, nurses, and other members of the health care team.
• Monitor patient progress and adjust the care plan as needed.
• Facilitate Interdisciplinary Team Meetings and ensure timely discharge planning.
• Negotiate approvals/authorizations for services and communicate with payer sources.
