Job summary

Alliance Health Services, Inc. is seeking an experienced RN for visiting-home health services. The role involves delivering patient care at home, conducting assessments, and coordinating with physicians and the agency to maintain optimal patient outcomes.

The ideal candidate will have an active CA RN license, BLS certification, and IV/phlebotomy credentials, with at least one year of home health experience. This position requires frequent travel and a per-visit pay structure.

Qualifications

• Active California RN license is required.
• BLS certification from American Heart Association or Red Cross required.
• IV/Phlebotomy certification required.

Responsibilities

• Provide nurse home visits to patients.
• Perform comprehensive initial assessments and document/upload to the Agency Portal.
• Obtain and document physician orders.
• Regularly update and revise the plan of care with the Clinical Team Manager.
• Coordinate care with physicians and home care team.
• Complete re-certifications, resumption of care, transfers and discharges.
• Assess needs for other team members (PT/OT/ST/MSW/HHA).
• Conduct Home Health Aide supervisory visits per regs.
• Demonstrate knowledge of regulatory requirements.

YOUR ROLE IN OUR COMPANY WILL BE

The RN travels to a patient's home to administer their services and helps patients maintain their independence. Their duties include: Administering at-home IVs, changing dressings, cleaning wounds, in-depth assessments, overseeing patient treatment plans, As well as Updating Doctors & The Agency about their patient's health. Documentation and communication with the Agency is done through Synergy Software.

AVAILABILITY

Our patients need care 7 days a week, although you have flexibility in your scheduling, we do ask that you are able to work at least two-three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer)

Work Area: City close to you, with a group of patients who need care for that day

RESPONSIBILITIES

• Providing nurse visits to home health patients. (home visits)
• Performing an initial, comprehensive assessment. Documenting everything & Uploading to Agency Portal
• Obtaining and documenting physician orders.
• In consultation with the assigned Clinical Team Manager, you will need to initiate and regularly re-evaluate and revise the plan of care
• Coordinating care with the assigned Clinical Team Manager, physician and other members of the home care team, informing them of significant changes in the patient's condition and needs.
• Performing re-certifications, resumption of care, transfers, and discharges as requested by the assigned Clinical Team Manager. Completing and submitting all related documentation
• Assessing the need for the services of other team members (PT, OT, ST, MSW, and HHA).
• Performing Home Health Aide supervisory visits per regulatory requirements.
• Demonstrate knowledge of regulatory requirements.

POSITION QUALIFICATIONS

• Visiting RN for Home Health: 1 year (Required)
• Nursing: 1 year (Required)
• License/Certification:
• Current CA RN License (Required)
• BLS from the American Heart Association and/or the American Red Cross (Required)
• NSO Nursing License Insurance (Required)
• IV/Phlebotomy Certified (Required)

PAY: $50.00 - $140.00 per visit
