Responsibilities

• Performs patient evaluations and collects history.
• Establishes an individualized plan of care addressing impairments.
• Conducts ongoing reassessment of patient progress.
• Initiates and manages discharge planning from first visit.
• Supervises and delegates to staff per state laws and policy.
• Documents clearly within policy and accreditation requirements.
• Communicates with patient, family, and health-care team to maximize care.
• Provides patient and family education to meet needs.
• Participates in professional development and quality improvement.
