Job summary
Creative Solutions in Healthcare, Inc. is seeking a Per Diem (PRN) Physical Therapist to join our in-house rehab team serving residents with person-centered care. You will assess, plan, and implement rehabilitation programs to improve or restore function, while coordinating with the interdisciplinary team and ensuring compliant documentation.

Qualifications
- Active Texas PT license in Physical Therapy.
- In good standing with regulatory agencies and licensing boards.
Responsibilities
- Consult with patients to learn about their physical condition.
- Assess and interpret patient evaluations and test results.
- Develop treatment plans using a variety of techniques.
- Create personalized rehabilitation programs for patients.
- Administer PT treatments to relieve pain and restore function.
- Ensure all treatment delivered aligns with the established plan of care.
- Monitor patient response and adjust approach with IDT collaboration.
- Provide clinical education to patients, families, and caregivers.
- Collaborate with the interdisciplinary team to ensure quality care.
- Ensure documentation is thorough and compliant with regulations.
- Attend required Rehab Director meetings.
