Job summary

Center of Illinois Home Healthcare, Inc. is a Medicare-certified home health agency seeking a Per Diem Physical Therapist to evaluate patients in their homes and develop individualized treatment plans. You will document care per CMS guidelines and collaborate with an interdisciplinary team.

Qualifications

• Current Illinois Physical Therapist license required.
• Minimum of one year home health experience preferred.
• Experience with Medicare home health regulations and documentation preferred.
• OASIS experience preferred.

Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans.
• Provide skilled physical therapy services in patients' homes according to physician orders and the plan of care.
• Assess patient progress and modify treatment plans as clinically appropriate.
• Educate patients, families, and caregivers on therapeutic exercises, mobility, fall prevention, and home safety.
• Complete timely and accurate documentation in accordance with Medicare, CMS, and agency guidelines.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, home health aides, and other members of the interdisciplinary team.
• Participate in care coordination and case conferences as needed.
• Maintain compliance with agency policies, state regulations, and accreditation standards.
