Overview

Lighthouse Hospice is currently hiring a Per Diem dedicated Hospice Nurse Practitioner to deliver exceptional patient care in Camden and Mercer counties. As a Hospice Nurse Practitioner, you will evaluate health histories, promote well-being, diagnose conditions, and develop care plans, collaborating with patients, families, and healthcare teams to ensure holistic care.

Responsibilities
  • Provide comprehensive medical care, manage terminal illnesses, and ensure compliance with state Nurse Practice Act under physician supervision.
  • Consult with physicians and healthcare practitioners for patient eligibility and ongoing care management.
  • Coordinate patient care with the interdisciplinary team ensuring a patient-centric approach.
  • Educate and counsel patients and families on care goals, addressing medical, psychological, and spiritual needs.
  • Assist in certification and recertification decisions for hospice care.
  • Maintain current knowledge of regulations and ensure compliance.
Qualifications
  • Master's degree with at least one year of hospice or palliative care experience preferred.
  • Current unencumbered RN and Nurse Practitioner licenses in the state of practice.
  • Specialist certifications (e.g., CHPN) preferred.
  • Excellent communication skills and proficiency in EMR systems.
  • CPR certification if required by state.
