Job Summary
Performs skilled nursing functions in accordance with federal, state, and local laws, professional organization guidelines, and Agency policies. All functions are performed according to established policies and the state Nurse Practice Act. The LPN/LVN delivers care as delegated by the RN Case Manager.
Job Duties
- Provides professional nursing care under the direction of a Registered Nurse and in accordance with the nurse practice act. Communicates client status/condition changes to the RN.
- Conducts skilled nursing visits as directed by the RN Case Manager, following the care plan established by the RN.
- Recognizes and reports changes in client condition to the Case Manager and physician.
- Communicates information promptly and accurately, verbally and through documentation in the clinical records.
- Scope includes personal care, monitoring, medication administration, client teaching, emergency intervention, and nursing services within professional scope and delegation.
- Promotes safety and a safe environment for clients and staff.
- Demonstrates knowledge of safety and infection control practices, complying with policies.
- Recognizes and responds to unsafe situations appropriately.
- Uses safe practices in all procedures and makes recommendations for improvements.
- Seeks education or assistance when needed.
- Performs duties in compliance with policies, procedures, and legal requirements.
- Maintains confidentiality and participates in Quality Improvement programs.
- Coordinates home care services, attends meetings, and completes mandatory training.
- Performs other duties as assigned.
Attendance Requirements
- Must be punctual and available for scheduled shifts.
Educational Requirements
- Graduate of an accredited nursing school.
Credential/Licensure Required
- Current LPN/LVN license in the state of employment.
- Current CPR certification.
- Valid driver’s license and appropriate auto insurance.
Experience Preferred
- At least one year in an acute care setting or equivalent.
Physical Requirements
- Medium exertion, lifting up to 35 pounds, involving sitting, walking, standing, driving, talking, hearing, reaching. Ability to work at a desk for up to eight hours and handle prolonged phone use.
Special Conditions of Employment
- Drug testing, ongoing exclusion, and disciplinary monitoring, and other eligibility requirements.

Compensation
$26.67 - $29.46 per hour. Final offer based on skills, experience, and education. The organization complies with all wage laws.
