Job description

Pay Rate: Starting visit rates at $35 - $37 per routine visit plus mileage.

Position Summary

Responsible to the Director of Professional Services and Clinical Manager to provide medically prescribed, goal-oriented therapy in accordance with the Physician Plan of Care, Company Policies, and professional Standards of Practice. Supervises personnel to deliver care and coordinates care delivery.

Job Responsibilities
  • Participate in developing patient physical therapy plans of care under the supervision of the Physical Therapist.
  • Make visits to patients and communicate with physicians as needed.
  • Follow the Plan of Care, document care accurately and submit electronic documentation timely to maintain patient medical records.
  • Provide education and instruction to team members, patients, and their families.
  • Participate in interdisciplinary team meetings to coordinate patient care, exchange information, problem-solve, and receive staff support and education.
  • Apply agency policies, procedures, and comply with Federal, State, and Accreditation standards in providing care.
  • Coordinate patient/family plans of care, review care plans regularly, and assess patient needs and requirements.
  • Complete electronic paperwork accurately and adhere to agency policies for timely submission.
Required Experience, Education, Skills & Abilities
  • Licensed as a Physical Therapy Assistant (PTA) in this state, in good standing with the Board.
  • Knowledge of principles, concepts, and methodology of physical therapy.
  • Ability to work well within an interdisciplinary team.
  • Ability to develop and maintain rapport with patients and families.
  • Excellent written and verbal communication skills.
  • Problem-solving and decision-making abilities.
Required Credentials
  • Current CPR certification.
  • Level 2 Background screening.
  • Recent negative TB / CXR.
