Job summary

Generations Healthcare is seeking a Physical Therapist for per diem, on-call and weekend coverage at our skilled nursing and rehabilitation facility. You will evaluate and treat residents, plan goals, educate families and document care according to policy, while advocating for patients.

Candidates must have an approved PT degree and active licensure, knowledge of Medicare and payors, and excellent English communication.

Qualifications

• Graduate of an approved curriculum in Physical Therapy.
• Licensed to practice in good standing.
• Knowledge of Medicare and payer sources.

Responsibilities

• Provide and direct all aspects of quality patient care, including evaluation, treatment and documentation.
• Develop treatment plans, set goals, and educate families.
• Serve as patient advocate and participate in quality assurance activities.
