Job summary

Village Point is seeking an experienced Per-Diem Physical Therapist to join our team at Village Point, a Springpoint Senior Living Community in Monroe Township, NJ. The therapist will assess, plan, organize, and administer rehabilitative programs to improve mobility, relieve pain, and enhance strength in residents.

The candidate has a NJ PT license and is committed to high-quality patient care, with responsibilities spanning exams, program design, documentation, and home-care instruction.

Qualifications

• High school diploma or equivalent required.
• Graduate of an accredited therapy program.
• Current NJ Physical Therapist State License required.

Responsibilities

• Performs and documents an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
• Plans and carries out individually designed physical therapy programs to improve functioning and relieve pain.
• Records prognosis, treatment, response, and progress in patient chart or computer system.
• Identifies goals and plans for reevaluation.
• Administers manual exercises, massage, or traction to relieve pain and improve strength.
• Tests strength, motor development, and functional capacity, recording data.
• Instructs patient and family in treatment procedures for home use.
• Confers with patient and medical team to plan and assess the program.
• Reviews physician's referral and medical records to determine needed therapy.
