Job summary

WeCare Centers at Jameson Care Center in New Castle, PA is seeking a Physical Therapist for per diem coverage. The role focuses on improving mobility and independence for residents through individualized therapy plans in a skilled nursing setting. New graduates are welcome and prior SNF experience is valued.

The therapist will implement treatment plans, educate patients and families, and document progress while collaborating with the care team to maintain quality outcomes.

Qualifications

• Valid state license or certification.
• Experience in skilled nursing preferred.
• New graduates welcome.
• Ability to communicate and instruct effectively.
• Hands-on experience with therapeutic exercises, functional training, and physical modalities.

Responsibilities

• Plan and administer treatment to residents and patients to relieve pain, restore or improve function, and promote healing.
• Ensure therapy is delivered according to an established plan of care.
• Provide clinical support and instruct patients, families, and caregivers.
• Monitor patient response to treatment interventions.
• Complete required forms and documentation per policy and regulations.
