Lexington Park Retirement Community in Topeka, Kansas, seeks a skilled Physical Therapist for a per diem role to support our rehabilitation team. You will evaluate residents, design and implement individualized treatment plans, and collaborate with a multidisciplinary team to maximize functional outcomes.

You will educate staff on PT modalities, document progress, and participate in interdisciplinary meetings.

Qualificações

• Licensed Physical Therapist in Kansas.
• Strong understanding of physical therapy principles and practices.
• Excellent communication and interpersonal skills.
• Ability to work in a fast-paced environment with minimal supervision.
• Proficiency in electronic medical records (EMRs).

Responsabilidades

• Provide physical therapy services to residents, including evaluations, treatments, and education.
• Develop and implement individualized treatment plans to promote resident independence and rehabilitation.
• Collaborate with other healthcare professionals, including speech therapy, occupational therapy, and nursing.
• Maintain accurate and up-to-date records of resident progress and treatment plans.
• Participate in resident care conferences and interdisciplinary team meetings.
• Provide education and training to staff on physical therapy modalities and techniques.
