Job summary

St Johnsbury Center for Living and Rehabilitation is seeking a Physical Therapist for a Per Diem position in a collaborative, patient-centered setting. You will assess needs, create and adjust treatment plans, and provide direct PT care to a diverse patient population.

The role emphasizes teamwork, clear communication with patients and families, and participation in wound care and prevention programs across shifts. Vermont licensure is required or in place.

Qualifications

• Degree in Physical Therapy from an accredited program.
• Current licensure in Vermont (state-specific).
• Ability to work independently and as part of a team.
• Excellent communication and interpersonal skills.

Responsibilities

• Assess patient needs and develop individualized treatment plans.
• Implement and modify treatment plans to achieve patient goals.
• Provide direct patient care including exercises, modalities, and manual therapy.
• Communicate effectively with patients, families and healthcare team members.
• Participate in wound care and pressure ulcer prevention programs.
• Collaborate with other healthcare professionals to achieve optimal outcomes.
