Job summary

St Johnsbury Center for Living and Rehabilitation is seeking an Occupational Therapist to join our per diem team in Vermont. You will assess patients, develop individualized treatment plans, and deliver therapy to improve function and independence.

Qualifications

• OT degree and state licensure to practice in Vermont.
• Experience developing individualized treatment plans.

Responsibilities

• Assess patients' physical and cognitive abilities and develop personalized treatment plans
• Provide occupational therapy sessions to improve patients' functional abilities and overall well-being
• Collaborate with interdisciplinary teams to ensure comprehensive care and achieve optimal patient outcomes
• Maintain accurate and detailed records of patient care and treatment plans
• Participate in quality improvement initiatives to continuously improve patient care and outcomes
