Job summary

Napa Post Acute in Arvin, CA is seeking a licensed Physical Therapist for per diem coverage in our skilled nursing facility. You will conduct evaluations, develop care plans, and deliver one-on-one therapy to post-acute patients to improve mobility and function.

Flexible shifts, competitive hourly rate of 65–75 USD, and a collaborative team environment are offered. CPR certification preferred, and geriatric rehab experience is a plus.

Qualifications

• CA Licensed Physical Therapist.
• Graduate of an accredited PT program.
• CPR certification required (or willing to obtain).
• SNF or geriatric rehab experience preferred.

Responsibilities

• Complete evaluations and develop care plans.
• Deliver 1:1 therapy per plan of care.
• Educate patients and caregivers on mobility and home exercise programs.
• Participate in care team meetings and discharge planning.
• Maintain accurate documentation and billing records.
