Job summary

Qualifications

• CPR certification is required or willingness to obtain.
• CA Licensed Physical Therapist credential required.
• Graduate of an accredited Physical Therapy program.
• SNF or geriatric rehab experience is preferred.

Responsibilities

• Complete evaluations and develop care plans.
• Deliver 1:1 therapy per plan of care.
• Educate patients and caregivers on mobility and home exercise programs.
• Participate in care team meetings and discharge planning.
• Maintain accurate documentation and billing records.
