Job Description

Evaluates, plans, directs, and administers individualized therapeutic treatment programs aimed at the restoration or improvement of function, or the prevention of disability. Writes evaluations, progress notes/reports, discharge summaries, and maintains patient files. Counsels patients and/or family members regarding rehabilitation diagnosis, plan of care, and prognosis. Maintains current knowledge of regulatory/legislative policies as they apply to the profession of Physical Therapy and adheres to same.

Qualifications

• Graduate of Accredited Physical Therapy Program
• Current NJ State License in Physical Therapy
• BLS (Basic Life Support) certification required at time of hire or within first 3 months of hire
• FIM (Functional Independence Measure) certification to be taken following employment and training at Wayne, then certification must be taken through the rehab program (Uniform Data System for Medical Rehab-UDSmr)
• For EIP (Early Intervention Program) only: Valid driver’s license required and comply with Qualifications to Drive a System Vehicle as well as all policies as outlined in the SJH Fleet Management & Safety Program
