Responsibilities

• Evaluate residents' physical abilities, functional limitations and mobility needs using standardized assessments.
• Develop and implement individualized treatment plans based on assessment results, resident goals and physician orders.
• Provide skilled physical therapy interventions, including therapeutic exercise, manual therapy and modalities.
• Monitor and document resident progress and adjust treatment plans as necessary.
• Communicate with interdisciplinary team members to coordinate care and ensure optimal outcomes.
• Educate residents and family members on physical therapy goals, treatment plans and home exercise programs.
• Maintain accurate and timely documentation of physical therapy services.
• Participate in team meetings, in-service education and quality improvement initiatives.
• Maintain current knowledge of physical therapy practice and relevant regulations.
