Job summary

Christian Health seeks a Per Diem Physical Therapist to join its Wyckoff outpatient rehab team. The role involves evaluating patients, designing and implementing individualized treatment plans, and collaborating with an interdisciplinary team to achieve optimal functional outcomes.

Qualifications

  • Must possess a current, unencumbered license to practice as a licensed Physical Therapist in the State of New Jersey.
  • CPR certification preferred.
  • Must be able to lift a minimum of 50 pounds without assistance.

Responsibilities

  • Provide and direct patient rehabilitation care within standard practice methods.
  • Evaluate patients’ needs and establish effective treatment plans.
  • Manage supervision of assistants and aides as directed.
  • Coordinate and document clinical and billing records.
  • Participate in weekly meetings and quality improvement activities.

We have an excited opportunity for Per Diem Physical Therapists to join our Outpatient Rehab at Christian Health. Our Outpatient Rehab offers expert physical, occupational, and speech therapy services conveniently located on our Wyckoff campus. We offer personalized treatment plans to help the patient move forward safely, confidently, and at their own pace. Our highly skilled interdisciplinary team of professionals are dedicated to a successful recovery and uses state-of-the-art, innovative approaches to help the patient achieve the best outcome. The Physical Therapist is responsible to manage the care of the patient, managing quality of all related outcomes. The Therapist participates as an integral part of the interdisciplinary team, manages resources to meet the needs of the patient balancing outcomes to provide effective and efficient treatment. Supervises clinically all assistants and aides of patients to which the Therapist is directing or overseeing the delivery of services according to an established plan of care. Supervision follows all accepted state and federal guidelines for the respective discipline. Demonstrates an understanding of, and embraces, the mission statement of Christian Health.

  • Provides and directs patient rehabilitation care within all general accepted practice methods, meeting all center, state, federal, and accrediting standards.
  • Evaluates patient's needs and establishes effective and efficient treatment plans for the patient assigned.
  • Manages and coordinates the supervision of treatment plans with all staff and other disciplines as indicated.
  • Participates in the screening of all patients according to directed schedules and practices.
  • Provides feedback to the designated supervisor on all clinical and rehabilitation issues.
  • Participates in all center directed processes and services.
  • Shows effective management of treatment and non-treatment related activities.
  • Participates in in-service training programs.
  • May travel or float to other CH communities to provide services.
  • Completes all necessary paperwork for clinical and billing documentation.
  • Treats patients in accordance with assignment and plan of care.
  • Participates in center and patient specific meetings as needed.
  • Assists in the maintenance and cleanliness of the department and equipment.
  • Provides supervision to assistants and aides/techs.
  • Completes weekly reports and patient summaries.
  • Participates in weekly meetings.
  • Updates and renews patient orders and plan of care as indicated.
  • Participates in monthly meetings.
  • Assures complete monthly billing.
  • Renews patient orders and updates plan of care as indicated.
  • Participates in quarterly quality improvement initiatives as Directed by DOR.
  • Participates in quarterly reviews of long-term residents as directed by DOR.
  • Participates in annual review.
  • Assists DOR in annual review of other staff as appropriate.
  • Performs other duties as required.
Qualifications:

Schedule:

Per Diem, Day shifts as needed Monday-Friday.

Education:
  • Graduate of an accredited college or university with a bachelor’s degree in physical therapy.
  • Must possess a current, unencumbered license to practice as a licensed Physical Therapist in the State of New Jersey.
  • CPR certification preferred.
