Job summary

Qualifications

• Pharm.D. from an accredited school of pharmacy.
• Current, valid West Virginia pharmacist license.
• Prior hospital pharmacy experience preferred.

Responsibilities

• Dispense medications as prescribed and counsel patients on drug usage, interactions, and side effects.
• Verify accuracy of prescriptions and assess appropriateness of therapy.
• Compound and dispense medications as required, overseeing measuring, weighing, and mixing.
• Maintain accurate pharmacy records, inventory, and controlled substance logs.
• Develop and implement procedures for preparation, packaging, labeling, storage, and handling of pharmaceuticals.
• Provide patient education and specialized services for conditions managed by the pharmacy.
• Collaborate with physicians and nurses to optimize medication regimens.
• Monitor medication use trends to identify safety risks and improve outcomes.
• Order and manage pharmaceutical supplies and ensure proper storage.
