Job summary

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for a Per Diem role delivering Home Health Visits in Chester County, PA. The OT will join a multidisciplinary team to provide rehabilitative care enabling patients to live safely at home.

Responsibilities include home visits, functional assessments, individualized treatment plans, and documentation. Per visit pay, with emphasis on quality care and patient independence.

Qualifications

• Current Pennsylvania Occupational Therapist license.
• Experience in home health preferred.
• At least one year of OT experience.
• Graduate from an accredited OT program (AMA/AOTA).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients’ functional status and ADL/IADL.
• Assess cognitive, psychological and motor aspects; develop intervention plans.
• Evaluate home environments for hazards and independence barriers.
• Identify equipment needs and environmental adaptations.
• Document observations and evaluations using a touch pad tablet.
