Job summary

OhioHealth in Pickerington, OH is seeking an Occupational Therapist to join our home health team. You will provide direct patient care in the home setting with autonomy and a fair weekend rotation.

Work with an interdisciplinary team to achieve patient outcomes, educate patients and families, and assist with discharge planning while maintaining safety and quality standards. Minimum qualifications include a Bachelor's degree in Occupational Therapy, Ohio license, and BLS; a valid driver's license

Qualifications

• Bachelor's degree in OT required.
• Ohio OT license required (Licensed Occupational Therapist).
• BLS certification.
• Valid driver's license required for home health assignments.

Responsibilities

• Direct patient care in home setting (90% of duties).
• Evaluate motor, cognitive, sensory, and psychosocial function.
• Develop and implement an OT plan of care based on evaluation findings.
• Educate patient and caregiver/family; coordinate discharge planning with the team.
• Supervise COTA and rehab technicians; participate in quality improvement.
• Attend multidisciplinary meetings; orient new staff and provide instruction.

Job Description Summary:

Join our amazing home health team where you have autonomy, a reasonable and fair weekend rotation, a flexible schedule, and limited on-call. Home health OTs provide skilled care to patients to achieve outcomes in collaboration with the patient's provider. There is an expectation that an employee in this position may be requested to work at care sites other than their home care site.

Responsibilities And Duties:

Driving is an essential function of the job.

• 90% Direct Patient Care - Evaluates and treats patients after physician referral to:
  • Evaluate patient's motor, cognitive, sensory integrative and/or psychosocial function.
  • Develops and implements plan of care based on evaluation findings and appropriate treatment techniques.
  • Implements plan of care based on appropriate occupational performance frame of reference(s): bio-mechanical, sensorimotor, and/or rehabilitative.
  • Attends multidisciplinary patient care meetings. Plans and coordinates discharge plan with multidisciplinary team.
  • Educates patient and caregiver/family.
  • Recommends Medical Equipment required for discharge.
  • Provides information to physicians, nurses, case managers, social workers, and other team members.
• 5%
  • Documents evaluation, plan of care, progress toward goals, discharge plan/continuity of care for multidisciplinary team, and other involved medical and insurance parties.
• 5% - Indirect Department Functions:
  • Supervises patient assignments and duties of Certified Occupational Therapy Assistants (COTA) and rehab technicians.
  • Is involved in department quality improvement through utilization review and quality assurance program.
  • Takes active role in orienting new staff members.
  • Serves as clinical instructor for OT and COTA students.
  • Educates other disciplines and medical/hospital personnel on the topic of OT purpose, goals and treatment policies and procedures.
  • Represents department at problem-solving and committee meetings.
  • Assumes other duties delegated by management.

Additional Job Description:

SPECIALIZED KNOWLEDGE

• Knowledge of home health Medicare regulations, JCAHO standards, home care reimbursement
• Experience in an acute, rehabilitation or home health setting
• Basic personal computer skills
• Knowledge in assessment, planning, implementation, and evaluation of occupational therapy
• BLS certification

For this position, in the Home Care department only, Minimum Qualifications include valid state driver's license, minimum of eighteen (18) years old, have an acceptable Motor Vehicle Report (MVR), and provide proof of insurance, if applicable.
