As a CCM Nurse Practitioner/Physician Assistant (per diem), you will deliver medical care services periodically or intermittently to Optum members.

Primary Responsibilities:

1. Primary Care Delivery:
   • Provide cost-effective, quality care to assigned members
   • Manage medical and behavioral, chronic and acute conditions, collaborating with physicians or specialists
   • Conduct comprehensive assessments, document findings per CMS regulations
   • Ensure accurate ICD10 coding and proper documentation of diagnoses
   • Address all quality elements and perform medication reviews and reconciliations
   • Engage with facility staff, families, and physicians to implement care plans
   • Evaluate and revise care plans as needed
   • Follow CCM practice guidelines and complete mandatory training
   • Travel between care sites is required
2. Care Coordination:
   • Understand and communicate benefits, policies, and procedures effectively
   • Assess and coordinate care during transitions across settings
   • Monitor and adjust care plans based on changing needs
   • Review interventions for appropriateness and effectiveness
   • Evaluate cost-effectiveness while meeting patient needs
3. Program Enhancement:
   • Maintain effective communication with all stakeholders
   • Promote CCM program growth in facilities
   • Innovate and improve care methods
   • Work independently with minimal supervision
   • Manage scheduling and participate in quality initiatives

Required Qualifications:

• Certified Nurse Practitioner with national certification (AANP or ANCC)
• Active, unrestricted license in your state
• DEA licensure or ability to obtain it post-hire
• Reliable transportation for site visits
• Physical ability to lift 30 pounds and navigate various dwelling conditions
• Availability for 24 hours/month, including off-hours
• Collaborative practice agreement, if required by your state

Preferred Qualifications:

• Over 1 year of post-grad experience in Long Term Care
• Knowledge of geriatrics, chronic illness, and end-of-life discussions
• Proficient in electronic medical records and building patient relationships
• Adaptable to change

The hourly pay ranges from $43.17 to $84.95, based on various factors including experience and location. Benefits include comprehensive packages, incentives, stock options, and 401k contributions.
