As a CCM Nurse Practitioner/Physician Assistant, you will deliver periodic or intermittent medical care to Optum members.

Primary Responsibilities:

• Primary Care Delivery:
  • Provide cost-effective, quality care to assigned members.
  • Manage medical and behavioral, chronic and acute conditions in collaboration with physicians or specialists.
  • Perform comprehensive assessments, document findings per CMS regulations.
  • Ensure accurate ICD10 coding and documentation of diagnoses.
  • Address all quality elements and conduct medication reviews and reconciliations.
  • Facilitate care plans with facility staff, families, and providers.
  • Evaluate and revise care plans as needed, using established guidelines.
  • Complete mandatory training and travel between care sites.
• Care Coordination:
  • Understand payer policies and communicate effectively with providers, members, and stakeholders.
  • Assess the necessity of ancillary services and coordinate member transitions across care levels.
  • Monitor member needs, review treatment responses, and optimize care plans for cost-effectiveness.
• Maintain effective communication with all parties, promote CCM programs, and participate in quality initiatives.
• Work independently, demonstrate initiative, and manage scheduling proactively.

Required Qualifications:

• Certified Nurse Practitioner with a national board certification.
  • MSN from an accredited program, certified by AANP or ANCC, with adult-gerontology, adult/family, or gerontology specialization.
• Active, unrestricted license in your state.
• Current DEA license or ability to obtain one.
• Reliable transportation for site visits.
• Ability to lift 30 pounds, navigate stairs, and adapt to varied living conditions.
• Ability to establish a collaborative practice agreement if required by your state.

Preferred Qualifications:

• Over one year of post-grad experience in Long Term Care.
• Knowledge of geriatrics, chronic illness, and end-of-life discussions.
• Proficient in electronic medical records and building positive relationships.
• Adaptability to change.

The hourly rate ranges from $43.17 to $84.95, depending on experience, education, and location. Benefits include comprehensive packages, incentives, stock options, and a 401k plan.
