Job summary

UnitedHealth Group's Optum Home & Community Care is seeking a Per Diem Nurse Practitioner to deliver comprehensive, patient-centered care for medically complex individuals in home and community settings. The role emphasizes health assessments, care management, and coordination with interdisciplinary teams to improve outcomes.

Serving the Bangor, Waterville, and Augusta region, the NP will focus on chronic disease management, preventive care, and care coordination with strong clinical support.

Qualifications

Current, unrestricted NP licensure in the state of practice.
National Board Certification as a Nurse Practitioner.
Active DEA license or ability to obtain.
Active driver's license and reliable transportation.
Ability to travel within the assigned territory.
Experience managing complex adult or geriatric patients.

Responsibilities

Provide care for medically complex patients in home and community settings.
Conduct comprehensive health assessments and ongoing care management.
Collaborate with interdisciplinary care teams and primary care providers.
Focus on chronic disease management, preventive care, and care coordination.
Provide care with autonomy and strong clinical support.
