Richmond Community Services seeks a Nurse Practitioner for a per diem role in Mohegan Lake, NY. Two positions are available to deliver person-centered care in residential and community settings across Westchester County.

Responsibilities include clinical assessments, treatment recommendations, and collaboration with nursing staff and physicians to ensure high-quality, coordinated care. NY NP certification, RN license, and prescriptive authority are required.

Competenze

Current New York State Nurse Practitioner certification and licensure.
Current, unrestricted Registered Professional Nurse (RN) license in New York State.
Prescriptive authority, as applicable to the position.
Strong clinical assessment, communication, and decision-making skills.
A compassionate, person-centered approach to care.
The ability to work collaboratively with an interdisciplinary team.
Experience supporting people with intellectual and developmental disabilities is a plus.

Mansioni

Conduct clinical assessments and evaluate health concerns for people we support in residential and community settings.
Provide recommendations for treatment, follow-up care, and ongoing health management within your scope of practice.
Collaborate with nursing staff, physicians, and other members of the care team to promote continuity of care.
Review medical concerns and help identify appropriate interventions to support each person's health, safety, and well-being.
Maintain accurate clinical documentation and ensure compliance with applicable healthcare regulations and professional standards.
